Fine Print for 4 Follow-Up Visit Package:

  • Package expires 12 months after date of purchase.

  • Includes four 30 minute Follow Ups. Extended Follow Ups (60 minutes) will utilize 2 Follow Ups.

  • A New Patient Appointment (purchased separately) is required before scheduling Follow Up Appointments.

  • Follow Up Package Appointments cannot be used with Dr. Tetlow or Dr. Patterson.

  • Not redeemable for cash. Non-transferable. Cannot be shared between patients.
